Ja, niżej podpisany/a rodzic lub opiekun prawny, wyrażam zgodę na udział mojego dziecka we wszystkich zajęciach kolonii letnich organizowanych przez Youth Camp Association District XII & XIII PNA w Yorkville.
Oświadczam, że zrzekam się wszelkich roszczeń, żądań oraz działań prawnych wobec Youth Camp Association District XII & XIII PNA, Zarządu, dyrekcji oraz personelu obozu z tytułu urazów, chorób lub innych zdarzeń, które mogą wystąpić podczas trwania kolonii, o ile nie wynikają one z rażącego zaniedbania.
W nagłych wypadkach wyrażam zgodę na to, aby lekarz lub uprawniona placówka medyczna zleciła wykonanie niezbędnych badań diagnostycznych (w tym prześwietleń), udzieliła rutynowej pomocy medycznej oraz podjęła leczenie mojego dziecka. W przypadku, gdy kontakt ze mną nie będzie możliwy, wyrażam zgodę na hospitalizację dziecka oraz przeprowadzenie koniecznych zabiegów medycznych, w tym podanie zastrzyków, znieczulenia lub wykonanie operacji, jeśli będzie to wymagane dla ochrony zdrowia lub życia dziecka.
Przyjmuję do wiadomości, że udzielenie pomocy medycznej nie stanowi przyjęcia odpowiedzialności przez organizatorów obozu ani zobowiązania do kontynuowania takiej opieki.
Rozumiem i akceptuję, że w przypadku poważnych problemów dyscyplinarnych dyrekcja obozu ma prawo, według własnego uznania, odesłać dziecko do domu bez obowiązku zwrotu kosztów obozu. W takiej sytuacji rodzic lub upoważniona osoba zobowiązana jest do niezwłocznego odebrania dziecka z terenu obozu.
Wyrażam również zgodę na wykonywanie zdjęć i nagrań z udziałem mojego dziecka podczas kolonii oraz na ich wykorzystanie w celach informacyjnych i promocyjnych organizatora, w tym na stronie internetowej oraz w mediach społecznościowych (np. Facebook, strona internetowa PNA Youth Camp).
CONSENT AND LIABILITY WAIVER
I, the undersigned parent or legal guardian, hereby give permission for my child to participate in all activities of the Youth Camp Association District XII & XIII PNA summer camp in Yorkville.
I hereby waive and release any and all claims, demands, or causes of legal action against the Youth Camp Association District XII & XIII PNA, its Board, directors, and staff for any injury, illness, or incident that may occur during the camp, except in cases of gross negligence.
In the event of an emergency, I authorize the attending physician or licensed medical facility to order necessary diagnostic tests (including X-rays) and to provide routine medical care and treatment for my child. If I cannot be reached in an emergency, I further authorize the hospitalization of my child and the administration of any medical procedures deemed necessary for the child's health or safety, including injections, anesthesia, or surgery.
I understand that the provision of medical or other services does not constitute an admission of liability or an obligation to continue such services by the camp or its representatives.
I understand and agree that the camp administration reserves the right, at its discretion, to dismiss my child from camp due to serious disciplinary issues, without any refund of camp fees. In such a case, the parent or authorized guardian is responsible for promptly picking up the child from the camp premises.
I also grant permission for photographs and/or video recordings that include my child to be taken during camp activities and used for informational and promotional purposes by the camp, including publication on the camp's website and social media platforms (e.g., Facebook, PNA Youth Camp website).

Camp Information & Rules
📅 Camp Session Schedule
- Each camp session runs from Sunday to Saturday.
- Drop-off: Sunday starting at 2:00 PM at the PNA Youth Camp Office.
- Pick-up: Saturday after concert at 10:30 AM.
🔄 Multiple Sessions
- Campers attending two consecutive sessions must be picked up on Saturday and returned on Sunday.
- Campers may not stay at camp over the weekend.
- In exceptional circumstances, an overnight stay may be approved for an additional fee of $100.
👨👩👧 Parent Visits & Early Pick-Up
- Parents may visit and temporarily take their child out of camp only after informing the counselor.
- A required release form must be signed before the camper leaves camp.
- All campers must be officially checked back in upon return.
📵 Communication & Electronics Policy
- Routine phone calls between parents and campers are strongly discouraged.
- Campers will not be removed from activities to take phone calls.
- Emergencies are the only exception.
- Cell phones are not allowed at camp.
- Any phone found will be confiscated and returned on the final day of camp.
- A $100 fine will be charged for any confiscated phone.
🚨 Health, Safety & Behavior
- Parents will be immediately notified by phone if any issue arises involving their child.
- Medication: Counselors must be notified upon arrival. All medication must be clearly labeled with the camper's first and last name and dosage instructions.
- Serious misconduct or repeated misbehavior will result in immediate dismissal from camp with no refund of camp fees.
🚗 Directions to Camp
From Chicago & North Suburbs:
Take I-294 to I-88 West → Route 56 West (Sugar Grove) → Route 47 South (Yorkville).
Before the bridge in Yorkville, turn right onto River Road. Camp is about ½ mile on the right.
From Chicago & South Suburbs:
Take I-55 South → Route 126 toward Plainfield → continue to Yorkville → Route 47 North.
After crossing the bridge, turn left onto River Road. Camp is about ½ mile on the right.
